Learn how to create and manage forms with the new forms feature.

As a coach, having efficient and effective tools at your disposal can make a significant difference in how you manage your practice and interact with your clients. Delenta's Forms module offers a versatile way to gather information, streamline client onboarding, and enhance communication.

You can easily create, customize and manage forms for your clients and team. Let's walk you through the process of creating a form, customizing it to meet your needs, sharing it with your clients, and managing responses.

Creating a Form

  1. Navigate to the Forms module from the main dashboard.

  2. Create a New Form

    • Click on the "Create New Form" button.

    • Select between create a blank form or use a template.

    • Enter a name for your form in the provided field.

Customizing Your Form

  1. Add Fields

    • You can add various types of fields to your form. Drag and drop the fields from the forms elements sidebar on you right and choose from the following options:

      • Header: Add a title or heading to your form. You can add only one of these to your form.

      • Subheader: Add a subtitle or use this to create multiple visual sections in you form.

      • Name: Add fields for first and last names.

      • Phone Number - Add a field to capture phone numbers.

      • Address: Include fields for street, city, state, and zip code.

      • Number: Add a field for numerical input.

      • Email Address: Include a field for email input.

      • Checkbox: Add a checkbox for multiple selections.

      • Radio Button: Include options for single selection.

      • Dropdown Field: Add a dropdown menu for predefined choices.

  2. Customize Field Content

    • After adding a field, click on it to customize its label and any additional options on the form designer panel on the left.

    • Use the drag-and-drop feature to rearrange the order of fields.

    • Click on Save button to save the form.

  3. Preview your form

    • Click on the preview button to see that your form is up to scratch. Now you're ready to publish and share.

    Pro Tip: You can set questions to Required, which will not allow users to complete the form without answering all the required questions.


Publishing Your Form

  1. Publish Settings

    • To publish your form, go to the “Publish” section.

    • Here you can change the name of you form. Just hit enter and you've saved the new name.

    • Now click on the publish button and you're good to go.

  2. Automation Triggers

    • Here, you can set up automation triggers to specify when and to whom the form will be delivered.

    • Choose the appropriate triggers based on your needs (e.g., before/after a discovery call/session for 1:1 sessions/group sessions etc).

    • Click "Save Changes".

Sharing Your Form

You will need to publish the form before you can share it.

  1. Share via Email

    • Once your form is ready, you can share it directly with clients onboarded on the Delenta system.

    • Navigate to the share settings and click on "Send via Email". Now select your client from the drop down you wish to share the form with.

    • Hit send and you're done.

  2. Share via Link

    • You can also generate a shareable link for your form.

    • Navigate to the share settings and click on “Share Link.”

    • Copy the link and share it with anyone via email or any other communication platform.


Managing Responses

When a form has been filled out by the recipient, the administrator or the sender of the form will receive both an email notification and an in-app notification alerting them that there are new responses to preview in the response section.

  1. Summary Section

    • Within the Responses tab, you may navigate to the Summary tab to view a summary of responses listed.

    • The summary section provides an overview of all responses, displaying aggregated data for easy analysis.

    • You can export the summary in .csv format.

  1. Individual Responses

    • To view responses on an individual basis, navigate to the “Individual Responses” section.

    • Here, you can see detailed responses from each recipient, allowing for a more in-depth review.
