The Forms module in Delenta's Team Portal is a powerful tool for team admins to create and manage forms for their coaching team. This guide will explain how team admins can create, publish, and manage forms, assign forms to clients, and view responses.
Accessing the Forms Module
Log In to the Team Portal
Log in to your Delenta account as an admin.
Navigate to the Forms module from the team portal dashboard.

Publishing and Sharing Forms
Publish the Form
Once your form is ready, go to the “Publish” section.
Here you can change the name of you form. Just hit enter and you've saved the new name.
Now click on the publish button and you're good to go.
Once published, these forms will be available for use to all your team coaches on their individual coach portal.
Automation Triggers
Here, you can set up automation triggers to specify when and to whom the form will be delivered.
Choose the appropriate triggers based on your needs (e.g., before/after a discovery call/session for 1:1 sessions/group sessions etc).
Click "Save Changes".
Share via Link
You will need to publish your form before you can share a link.
As an Admin you'll only be able to share the form with a shareable link.
Navigate to the share settings and click on “Copy Link.”
Copy the link and share it with your coaches or their clients.
Assigning Forms to Clients
Assign Forms via Tasks and Goals
Team admins can assign forms to clients by linking them to specific tasks and goals.
Select the client, go to the Tasks and Goals section and attach the form.
Managing Forms and Responses
Forms created in the team portal will only be accessible to the team admins.
Edit and Delete Forms
Only team admins will be able edit or delete these forms. Coaches can view but cannot modify forms in the Coach Portal.
To edit or delete a form, go back to the Forms module in the Team Portal, select the form, and choose the appropriate action.
View Responses
Responses can be viewed in the "Responses" section of the form.
Summary View: Provides an overview of all responses, displaying aggregated data.
Individual View: Allows you to see detailed responses from each recipient.
Export Responses
You can export responses for further analysis or record-keeping.
In the "Responses" section, click on the export button to download the data in your preferred format.
By following these steps, team admins can effectively create, customize, publish and manage forms within the Team Portal, ensuring a smooth workflow and enhanced communication with both coaches and clients.
